Parish Registration Form

Aloha and welcome!  We invite you to become a registered member of the St. Anthony parish 'ohana. 


There are many reasons to register as a parishioner of St. Anthony of Padua Catholic Church:

  • A sense of belonging in our Catholic community.
  • We can effectively communicate with you and provide you up-to-date information on our parish.
  • Have the support of a faith family.
  • Registration for Faith Formation classes and programs.
  • Eligibility to receive the Sacraments of Baptism, First Reconciliation, First Holy Communion, Confirmation and Holy Matrimony

Mass Intention Form

Mass intentions are a form of almsgiving through which parishioners contribute to the good of the church by supporting its ministers and its works.


Parishioners who make the monetary offerings for Mass intentions, are making a tremendous sacrifice. While it may seem on the surface just a financial contribution, the personal sacrifice draws a person closer to the theological depth of the Mass.


In the Diocese of Honolulu, the standard amount set for Mass donations is $10. Although parishioners can give more at their discretion, $10 was the figure determined appropriate by the bishops of the San Francisco Province — of which the Honolulu diocese is a part — in 1997.

Our goal is to provide individuals with an environment where you can build and grow your relationship with God, know and love Him, and live as His disciples in our community.  We provide youth, teen, young adult, and adult formation and ongoing education classes.

Baptismal Form

Aloha, Parents and godparents/sponsors,

Welcome to Baptism – the sacrament that opens the door of faith to your child and into the Roman Catholic Church. The baptism of your child offers a great opportunity to deepen your appreciation for and experience of the faith.


Therefore at St. Anthony Church in Wailuku, there is a mandatory Baptism Class that both parents and godparents must attend. The Baptism Class is to help prepare you and help you understand more about the sacrament. The class helps clarifies each person’s role in the child’s faith formation and spiritual growth. Thus, why it is very important that parents and godparents make time for this class.

Baptismal Godparent(s) Form

Qualifications: Godparents/sponsors not only serve as witnesses at the ceremony of Baptism but also assist the person in leading a Christian life. Therefore, Church law requires that godparents / sponsors be people who are living the Catholic faith and meet the following qualifications (Code of Canon Law, canons 872-874)

  • They should ordinarily be at least 16 years of age.
  • They must be Catholics who have received Baptism, Confirmation and First Holy Communion.
  • They must lead a life in harmony with the Catholic faith.
  • If they are married, they must be married in the Catholic Church.
  • Parents of the person to be baptized may not serve as godparents/sponsors.

Notes:

  • Not qualified? Please speak to your pastor about receiving the sacrament and getting qualified.
  • Christian Witnesses for Baptism: If only one godparent is chosen for Baptism, someone validly baptized in a non-Catholic ecclesial community may serve as a Christian witness to the ceremony, along with the Catholic godparent. This form is not required for Christian witnesses. (Baptized Catholics who no longer practice their faith or who have joined another ecclesial community may not serve as Christian witnesses.)
  • A non-baptized person cannot be a witness. (Code of Canon Law, 872-874)
  • Orthodox Christians: Orthodox Christians may serve as godparents/sponsors like a Catholic.
  • Proxies: If the godparent/sponsor cannot be present for the ceremony, a proxy may stand in as a witness at the ceremony.  This form is not required for proxies.

Facilities Use Forms

For parties requesting use of St. Anthony facilities, please use the following forms:

  • Form A: For one time events.  Events are short in duration from one day to a week long.  This does not cover repititous events.  Examples: fundraisers, craft fairs, food truck event, special event parking, sporiting events, community events, or TV/movie taping.
  • Form B: For short term use events.  Events that are repetitive and have a duration of several days to less than 1 year. Examples: cafeteria use (zumba, martial arts, music and dance classes), general meetings, summer  programs, farmers markets, parking lot rental, commercial kitchen use.

Submit completed forms to the church office either in person or via email (sacmaui@rcchawaii.org) at least one month prior to the event.
